Clinical and genetic diagnosis and management of rare genetic disorders

Absztrakt

Introduction: 80% of rare diseases have a genetic origin, 50% manifest as congenital anomalies. Their adequate health care includes early recognition of genetic anomalies and prevention of recurrence. Objectives: The authors’ aim was to provide correct diagnoses to patients with multiple congenital anomalies with or without mental retardation attending to the outpatient clinic of the Clinical Genetics Center at the University of Debrecen in the time interval of Aug.01.2007- March 31.2013., establish the possibility of prenatal diagnosis, assess the distribution of different genetic mechanisms in the background of rare genetic diseases, compare them with international data and develop an algorithm for the diagnostic approach of rare genetic diseases applicable in Hungary. Methods: Clinical data and genetic results of patients were evaluated, and patients were categorized into one of the ten proposed etiological groups, based on which the distribution of genetic causes were defined. Results: Clinical diagnosis was achieved in 64.3% of patients, confirmed genetic diagnosis in 37.8%, and 35.7% of patients remained undiagnosed. Several dysmorphic syndromes and metabolic disorders were first diagnosed in Hungary, two of which being unique in the literature. Conclusion: In our center the diagnostic yield of chromosome-aberrations exceeds the international standards, that of known microdeletions and dysmorphic syndromes meet international data, and the genetic diagnosis of mendelian disorders and submicrosopic copy number changes remains below international figures.

Bevezetés: A ritka betegségek 80%-a genetikai eredetű, 50%-uk veleszületett rendellenesség formájában manifesztálódik mentális retardációval vagy anélkül. Ellátásukban a genetikai eltérések korai felismerésére, és a prevencióra kell törekednünk. Célkitűzés: A Debreceni Egyetem Klinikai Genetikai Központja járóbeteg rendelésén 2007.aug.01- 2013.márc.31. között megjelent veleszületett rendellenességes és/vagy mentálisan retardált betegek pontos diagnózisának felállítása, a prenatális diagnózis lehetőségének biztosítása, az eltérő genetikai csoportok megoszlásának felmérése és nemzetközi eredményekkel való összehasonlítása, a ritka genetikai betegségek hazai vizsgálatára alkalmas diagnosztikus útmutató kidolgozása. Módszer: A betegeket klinikai adataik és genetikai eredményeik alapján tíz etiológiai csoportba soroltuk. Eredmények: Klinikai diagnózist az esetek 64.3%-ában, genetikailag igazolt diagnózist 37.8%-ban igazoltunk, míg 35.7%-ban a diagnózis tisztázatlan maradt. Diagnosztikus munkánk során több ritka dysmorphia-szindróma és anyagcserebetegség került – Magyarországon elsőként – felismerésre, két ritka kórkép a világirodalomban is egyedülállónak számít. Következtetések: Központunkban a diagnosztikus hatékonyság kromoszóma rendellenességek esetén meghaladja, ismert mikrodeléciók és dysmorphia-szindrómák esetén eléri a nemzetközi mutatókat, míg mendeli betegségek és szubmikroszkopikus kópiaszám-változások esetén elmarad azoktól.
