Új generációs β-ciklodextrinek sejtmembránra, illetve a taxol, mint BCS IV. típusú farmakon transzportjára gyakorolt hatásának in vitro vizsgálata

Absztrakt

A gyógyszer bejutásának növelése, különösen a Biofarmáciai Osztályozási Rendszer IV. osztályába tartozó hatóanyagok esetén, mint például a taxol, rendkívül fontos feladat. A farmakonok felszívódásának in vitro vizsgálatával meghatározhatóak azok a tényezők, amelyek modulálásával a biofarmáciai tulajdonságok előnyösen megváltoztathatóak. A gyógyszerforma kialakítása során felhasznált olyan speciális segédanyagok, mint például a ciklodextrinek (CD) rendkívüli módon befolyásolják a hatóanyag szervezetbe történő bejutásának módját és mértékét, befolyásolják a biológiai hasznosíthatóságot. Ezért fontos ismernünk a segédanyagoknak a felszívódásra gyakorolt hatását és az esetlegesen előforduló toxicitási tulajdonságaikat. A vizsgálataink megtervezésekor ezeket a szempontokat figyelembe véve, a következő célokat tűztük ki: 1. Caco-2 sejtek segítségével kialakított in vitro transzport modell beállítása, jellemzése TEER méréssel, illetve ismert permeabilitású vegyületek segítségével: A Debreceni Egyetem Gyógyszertechnológiai Tanszékén beállítottuk, jellemeztük a Caco-2 sejtekkel kialakított bélhám modellt. Ez a modell alkalmas a felszívódás in vitro szimulálására, segítségével tanulmányozható a transzportfolyamatok széles spektruma. 2. Új generációs metil-β-CD-ek citotoxicitásának, hemolitikus aktivitásának jellemzése: Az új generációs metil-β-CD-ek az anyavegyületekhez képest jelentősen kisebb citotoxikus hatást fejtenek ki. A koleszterinnel töltött CD-ek esetében nem tapasztalható citotoxicitás. Ezáltal kísérletes úton igazoltuk, hogy a sejtkárosító hatás összefügg a membránból történő koleszterin kivonással. A hemolitikus aktivitási tesztek eredményei összhangban állnak az MTT-teszt eredményeivel. 3. Összefüggések feltérképezése a vizsgált CD-ek szerkezete, koleszterinoldó képessége, citotoxicitása, illetve hemolitikus aktivitása között: Szerkezet-hatás összefüggés elemzése során megállapítottuk, hogy a metil-β-CD származékok citotoxicitása, hemolitikus aktivitása és koleszterinoldó képessége jelentős mértékben függ az ionos (anionos vagy kationos) szubsztituensek jelenlététől, a metil csoportok számától, viszont lényegében független a pozíciójuktól. A kísérletek alapján szoros korreláció állapítható meg a koleszterinoldó képesség és a citotoxicitás, illetve a koleszterinoldó képesség és hemolitikus aktivitás között, mely alapján kijelenthető, hogy a koleszterinoldó képesség prediktív faktorként utal a CD sejtkárosító hatására. 4. A citotoxicitási és koleszterinoldó képesség alapján legmegfelelőbbnek ítélt CD-ek taxol transzportjára gyakorolt hatásának tanulmányozása Caco-2 transzport modellen: A toxicitási profil alapján további vizsgálatokhoz kiválasztottunk három CD-t, a Rameb-et, MaRameb-et, SuRameb-et. Megállapítottuk, hogy a kiválasztott CD-ek képesek a taxol permeabilitását fokozni, melyhez állandó CD jelenlét szükséges, taxol-CD komplex formában. Ennek hátterében az oldékonyság növelése, illetve a CD-ek sejtmembránnal történő kölcsönhatása áll. 5. A kiválasztott CD-ek a taxol aktív effluxára, illetve a barrier funkciókra gyakorolt hatásának tanulmányozása: Az ismert P-glikoprotein inhibitor, a ciklosporin A hatásával eltérő módon, a CD-ek nem befolyásolták 20 mM koncentrációban az aktív efflux mechanizmusok aktivitását. Rameb esetében elmondható, hogy a permeabilitás fokozódás összefügghet a barrier funkciók csökkenésével. A Rameb ionos származékai a monolayer barrier funkcióját és a szoros junkciókat alkotó egyes fehérjék internalizációját sem befolyásolták a Rameb-hez hasonló mértékben, ám mégis jelentősen növelték a taxol monolayeren történő átjutását. Mindent egybevetve a MaRameb bizonyult a legalkalmasabbnak a vizsgált β-CD-ek közül a taxolból készítendő per os gyógyszerforma formulálására. The increase of drug permeability especially in case of Biopharmaceutical Classification System Class IV. drugs, for example taxol, is a really important task. By the in vitro investigation of pharmacon absorption those factors can be determined, of which modulation the biopharmateutical properties can be positively changed. As well the properties of the applied special excipients, for example cyclodextrins (CDs) are tremendously affecting the entrance way of the drug and the amount of penetrated drug. Therefore it is important to know the effect of excipients on the absorption and their possible toxic properties. While planning our experiments these aspects were considered and the following aims were settled: 1. Installation of the Caco-2 cell in vitro transport model and its characterization with TEER measurements and also with known permeability drugs: A Caco-2 transport model was installed and characterized at the Department of Pharmaceutical Technology, University of Debrecen. The Caco-2 intestinal model is widely used for in vitro simulation of drug absorption, and is an appropriate model for studying the different ways of absorption. 2. Citotoxicity and hemolytic activity investigation of the new-generation methylated-β-CDs: The new generation CD derivatives showed lower cytotoxicity than the parent methylated compounds. The complexation of cholesterol into the cyclodextrin ring dramatically decreased the cytotoxicity of CDs on Caco-2 cells. The cytotoxicity of various CD derivatives depends on their cholesterol extraction capacity from the cell membrane. Hemolytic activity test results were in accordance with MTT test results. 3. Exploring the coherences between the structure, the cholesterol solubilizing capacity, the citotoxicity and the hemolitic activity of the investigated CDs: From structure-activity analysis it can be concluded that in the case of methylated-β-CD compounds the cytotoxicity depends highly on the number and slightly on the position of methyl groups. Significant correlation can be observed between the cytotoxicity, hemolytic activity and the cholesterol complexing capacity of various CD compounds. I propose that cholesterol-solubilizing properties can be a predictive factor for β CD cell toxicity which depends on the structure of the CD derivative. 4. Investigating the effects of best-choice CDs based on citotoxicity and cholesterol solubilizing capacity on the transport of taxol in the Caco-2 transport model: Based on their cytotoxicity profile, three CD derivatives were chosen: Rameb, MaRameb, and SuRameb. I have demonstrated that Rameb and its less toxic ionic derivatives MaRameb and SuRameb effectively increase taxol permeability in Caco-2 monolayers, but the continuous presence of these CD derivatives is necessary for the enhanced taxol permeability in taxol-CD complex form. This effect could primary be mediated by increased complexation and solubility of taxol and also possibly by the interaction of CDs with cell membrane. 5. Investigating the effects of chosen CDs on the active efflux of taxol and on the barrier functions: In contrast to the well-known active efflux pump inhibitor cyclosporine A, the examined CD derivatives at 20 mM concentration did not influence the activity of active efflux pumps. In the case of Rameb, the increased paracellular permeability can indicate attenuated barrier integrity. However the ionic derivatives of Rameb did not change the barrier function of the Caco-2 monolayer and did not induce a drop in TEER or internalization of specific tight junction proteins, they are able to enhance the taxol influx in Caco-2 cell monolayer. MaRameb proved to be the most suitable β-CD in oral formulation of taxol.
