Vascularis, immunológiai és genetikai tényezők lehetséges patogenetikai szerepe néhány gasztroenterológiai kórképben

Absztrakt

A coeliakia és a gyulladásos bélbetegségek (IBD) változatos klinikai megjelenését befolyásoló faktorok jelentős része ismeretlen. A haptoglobin (Hp) molekula három fő fenotípusa különböző mértékű antioxidáns, scavenger és immunmoduláns tulajdonsággal rendelkezik, és ezek kapcsolatot mutatnak egyes gyulladásos betegségek kórlefolyásával. A Hp α-lánc 1-1 genotípusa kis molekulasúlyú dimerek, a 2-1 genotípusa különböző hosszúságú lineáris polimerek, a 2-2 genotípusa ciklikus polimerek képződését eredményezi. Munkánk során vizsgáltuk a Hp polimorfizmus megoszlását, valamint annak lehetséges kapcsolatát a betegség klinikai megjelenésével nagyszámú coeliakiás és IBD betegben. A Hp fenotípusokat nátrium-dodecil-szulfát poliakrilamid gélelektroforézissel (SDS-PAGE) és immunblottal határoztuk meg, mely egyértelműen azonosítja a betegek genotípusát is. Az eredményeket az egészséges magyar népességet reprezentáló kontrollokhoz viszonyítottuk. Coeliakiás betegekben a Hp2-1 típus előfordulása szignifikánsan gyakoribb, míg a Hp2-2 típusé ritkább volt az egészséges populációban találtakhoz képest. A Hp2-2 fenotípusú személyeknél azonban a súlyos, malabszorpcióval jelentkező klinikai betegség kockázata magasabb volt, a silent betegségé viszont jóval alacsonyabb. Dermatitis herpetiformisban a Hp fenotípusok megoszlása nem tért el a coeliakiás csoporttól. A Hp fenotípusok megoszlása IBD-ben nem különbözött az egészséges populációban észlelttől. Ugyanakkor Crohn-betegekben a Hp2-1 fenotípus esetén a gyulladásos (nem stenotizáló, nem penetráló) betegségforma előfordulása szignifikánsan gyakoribb volt a másik két Hp fenotípushoz viszonyítva, míg a stenotizáló forma előfordulása ritkább. Továbbá az extraintesztinális manifesztációk közül primer szklerotizáló cholangitisben a Hp1-1 fenotípus egyáltalán nem fordult elő. A Hp szerepét a molekula immunreakciókban betöltött moduláló hatásai és eltérő térszerkezete magyarázhatják.
A mikrobiális- és autoantigének elleni szerológiai válasz, mint a Saccharomyces cerevisiae (ASCA) és Escherichia coli külső membrán porin C elleni antitestek (anti-OmpC) vagy a perinukleáris antineutrofil citoplazmatikus antitestek (P-ANCA) segítséget jelenthetnek az IBD differenciál diagnosztikájában és újabb adatok szerint összefüggést mutathatnak a betegség fenotípusával. Munkánk során nagyszámú IBD betegben és egészséges kontroll egyénekben vizsgáltuk a fenti antitestek előfordulási gyakoriságát és azok lehetséges kapcsolatát a betegség klinikai megjelenésével, lefolyásával, az extraintesztinális tünetekkel és a veleszületett („innate”) immunitásban szerepet játszó receptorok mutációival és polimorfizmusaival. Vizsgáltuk továbbá az antitestek szerepét a kezelés hatékonyságának előrejelzésében. Értékeltük az etanol-fixált neutrofil granulocyta preparátumokkal párhuzamosan használt formalin-fixált szubsztrátok alkalmazhatóságát az IBD asszociált atípusos P-ANCA mintázat azonosításában. A módszer sorozatok (inter-assay) és vizsgálók közötti (inter-observer) variabilitását az egyes mintáknak négy különböző kereskedelmi forgalomban elérhető assay felhasználásával, két különböző laboratóriumban történő tesztelésével határoztuk meg. Az ASCA és az anti-Omp meghatározás szérum mintákból történt enzimmel kapcsolt immunoszorbens assay-vel (ELISA), az ANCA meghatározás pedig indirekt immunfluoreszcens (IIF) vizsgálattal. A nucleotid oligomerisatios domain/caspase recruitment domain (NOD2/CARD15) mutációkat és a toll-like receptor (TLR)-4 D299G variánsokat polimeráz láncreakció/ restrikciós fragmens hossz polimorfizmus (PCR-RFLP) vizsgálat segítségével határoztuk meg. A szerológia markerek hasznosnak bizonyultak a Crohn-betegség és a colitis ulcerosa differenciál diagnosztikájában. Az ASCA és az anti-Omp antitestek jelenléte Crohn-betegség, míg az atípusos P-ANCA pozitivitás colitis ulcerosa fokozott kockázatával járt. Az etanol-fixált neutrofil granulocyták mellett alkalmazott formalin-fixált preparátumok azonban IBD-ben nem segítették a P-ANCA és az atípusos P-ANCA elkülönítését. A különféle P-ANCA mintázatok együttes értékelése a Crohn-betegség és a colitis ulcerosa differenciál diagnosztikájában a specificitás és szenzitivitás növekedését eredményezte. Továbbá csökkentette a különféle kereskedelmi forgalomban elérhető tesztek inter-assay és az inter-observer variabilitását. Crohn-betegségben az ASCA és az anti-Omp pozitivitás a vékonybél érintettség és a nem-gyulladásos betegségforma szempontjából a logisztikus regressziós analízisben is független rizikófaktornak bizonyultak, míg a műtéti kockázat, a szteroid és infliximab kezelésre adott válaszkészség tekintetében nem. A szerológiai válasz vonatkozásában nemcsak kvalitatív, hanem kvantitív összefüggés is kimutatható volt. A nem-gyulladásos típusú vékonybél Crohn-betegség és az antimikrobiális antitest pozitivitások száma, illetőleg az antitest titerek között szintén pozitív korrelációt találtunk (szerológiai-dózis hatás). Az ASCA és az anti-Omp antitestek jelenléte kapcsolatot mutatott a NOD2/CARD15 genotípussal. Az antimikrobiális antitestek előfordulási gyakorisága szignifikánsan nőtt a NOD2/CARD15 mutációk számának növekedésével, mely gén-dózishatásra utal és alátámasztja a mikrobákkal szembeni megváltozott érzékelés szerepét a Crohn-betegség patogenezisében.

Clinical presentations of the celiac disease and inflammatory bowel diseases (IBD) are highly variable but little is know about those factors determining disease phenotype. Since differences in the antioxidant, scavenging and immunomodulatory properties were found among three major haptoglobin (Hp) phenotypes, Hp polymorphism was reported to be associated with the risk and clinical course of inflammatory diseases. The 1-1 genotype of Hp α-chain results in production of low molecular weight dimers, while the 2-1 and 2-2 genotype are associated with variable length of linear polymers and cyclic polymers, respectively. The aim of our study was to investigate the distribution of Hp polymorphisms in large cohort celiac patients and in patients with IBD, and also their possible association with the clinical presentation of these diseases. Hp phenotypes were determined by sodium-dodecyl-sulphate polyacrylamide gel electrophoresis (SDS-PAGE) and immunoblotting of the sera, which clearly identifies the genotype of the patients. The Hp results were compared to a group of healthy individuals representing the normal Hungarian population. In celiac patients, the frequency of Hp2-1 phenotypes was significantly higher compared to the control population. The occurrence of Hp2-2 phenotype was lower; however patients having this phenotype were at an increased risk of severe malabsorption as a clinical presentation of the disease but reduced risk of silent disease. Patients with dermatitis herpetiformis showed similar Hp phenotype distribution to those in the whole celiac patients group. The Hp phenotype distribution showed no difference in IBD group as compared to control population. In Crohn’s disease, patients with Hp 2-1 type carried a higher probability for inflammatory (non-stricturing, non-penetrating) form compared to the other two phenotypes, while the stricturing form developed less frequently. Regarding extraintestinal manifestations of the disease, we found no Hp1-1 expression in patients with primary sclerosing cholangitis. The role of Hp molecules may be contributed to their distinct immunomodulatory properties and structural characteristics. Sero-reactivity to microbial components like Saccharomyces cerevisiae (ASCA) and outer membrane porin protein C of Escherichia coli (anti-OmpC) or autoantibodies directed against perinuclear components of neutrophils (P-ANCA) are reported to be associated with disease phenotype and may be of diagnostic importance in IBD. The aim of our study was to investigate the prevalence of serological markers in a large cohort of IBD patients and healthy controls. We also assessed the possible interaction with the clinical presentation, response to treatment and extraintestinal manifestations. Additionally, we studied the relationship between serological response and genetic factors. We analyzed the feasibility of using the combination of ethanol-fixed and formaldehyde-fixed neutrophil substrates in the identification of IBD associated atypical P-ANCA. We also examined the inter-assay variability of this method by comparing four different commercially available assays, and the inter-observer precision based on the results of two distinct laboratories. Sera were assayed for ASCA and anti-Omp by enzyme-linked immunosorbent assay (ELISA) and ANCA by indirect immunofluorescence (IIF) method. The nucleotide-binding oligomerization domain/ caspase recruitment domain (NOD2/CARD15) and the toll-like-receptor (TLR)-4 variants were tested by polymerase chain reaction/ restriction fragment length polymorphism (PCR-RFLP). Serological markers were useful in the differentiation between Crohn’s disease and ulcerative colitis. ASCA and anti-Omp positivity were associated with increased risk for Crohn’s disease. In addition, atypical P-ANCA was associated with increased risk for ulcerative colitis. The combined application of ethanol-fixed and formaldehyde-fixed substrates did not facilitate the differentiation between P-ANCA and atypical P-ANCA, and the results were not consistent when using substrates from different sources. Combining all P-ANCAs (typical and atypical) together ensures the highest sensitivity and specificity in differentiating ulcerative colitis from Crohn’s disease. In a logistic regression analysis, ASCA and anti-Omp were independently associated with ileal and non-inflammatory disease, but not with a risk for surgery or response to steroids or infliximab. In addition to qualitative correlations, quantitative correlations with serologival response were also present. The number of antibodies produced against microbial antigens and their titers in Crohn’s disease showed a positive correlation with the small bowel involvement and the severity of the disease course (serology dosage effect). Moreover, reactivity to microbial components was associated with NOD2/CARD15 genotype. Positive correlation was found between the number of mutations and the prevalence of antimicrobial antibodies (gene dosage effect), further supporting the role of altered microbial sensing in the pathogenesis of Crohn’s disease.
