Területi és nemi inhomogenitás az emlős kamrai szívizomszövetben

Absztrakt

Célunk az egyes ionáramok, és az azokért felelős csatornafehérjék eloszlásának vizsgálata volt kutya és humán kamrai szívizom epikardiális (EPI) és midmiokardiális (MID) rétegeiben, illetve az apikális (APEX) és bazális (BASE) régióiban. Összehasonlítottuk a kutya és humán sejteken elektrofiziológiai és immunhisztokémiai módszerekkel kapott eredményeket. Ezen kívül a tesztoszteron és ösztrogén hatását vizsgáltuk az EKG paraméterekre és a szívizom ioncsatorna expressziójára hím és nőstény kutyákban. Vizsgálatainkhoz konvencionális mikroelektródatechnikát, egészsejt patchclamp és Western blot módszert használtunk. Az EKG vizsgálatokat altatott hím és nőstény kutyákon végeztük kasztráció előtt, után és inverz hormonszubsztitúciót követően. Eredményeink alapján az akciós potenciál időtartama rövidebb, a korai repolarizáció amplitúdója pedig nagyobb volt az APEX és az EPI eredetű kutya kamrai sejteken, mint a BASE és a MID sejteken. Az akciós potenciál egyéb paramétereiben ill. a sejtek kapacitásában nem volt szignifikáns különbség. Feszültség-clamp körülmények között vizsgálva a tranziens kifelé irányuló K+- áram (Ito) és a késői egyenirányító K+-áram lassú komponensének (IKs) amplitúdója APEX és EPI sejteken közel kétszer nagyobbnak adódott, mint a BASE és MID sejteken. Nem találtunk viszont szignifikáns különbségeket a többi vizsgált ionáram (IKr, IK1, ICa) amplitúdójában és kinetikai paramétereiben. Az immunhisztokémiai vizsgálatok eredményei – az ionárammérések adataival teljes összhangban – csak az Ito és IKs áramokért felelős csatornafehérjék esetében mutattak intenzívebb apikalis expressziót: a Kv1.4, KChIP2, KvLQT1 és MinK alegységek denzitása apikálisan nagyobbnak bizonyult, mint bazálisan. Az EPI minták esetében a Kv4.3, a Kv1.4, a KChIP2 és a KvLQT1 proteineknek az expressziója volt magasabb a MID sejtekhez képest. Hasonló ioncsatorna expressziós mintázatot kaptunk a humán szívizom minták esetében is, mint amit kutya mintákban észleltünk, kisebb mennyiségi különbségeket leszámítva. A nemi különbségek vizsgálatakor a szívfrekvencia csökkent, a pitvar-kamrai átvezetési idők pedig emelkedtek kasztráció hatására mindkét nemben, miközben az invery hormonszubsztitúció visszaállította az eredeti értékeket. Orchiectomia 53 szignifikánsan emelte a QT- és QTc-időt és a QTc-diszperziót, miközben a kasztrált nőstények tesztoszteron kezelése ellentétes hatású volt. Az IK1 és Ito áramokért felelős ioncsatorna-fehérjék (Kir2.1 és Kv4.3) expressziója magasabb volt a normális hím és a tesztoszteron-kezelt kasztrált nőstény egyedekben. A tesztoszteron szignifikánsan befolyásolta, ösztrogén viszont nem hatott a kutya kamrai szívizom repolarizációjára, nemtől függetlenül. Ez a hatás valószínűleg a K+-csatornafehérjék expressziójának fokozásán keresztül valósul meg. A kutya és humán kamrai szívizomban talált regionális és nemi eltérések mind a diszperzió, mind pedig a gyógyszerhatások szempontjából figyelmet érdemelnek. Vizsgálataink alapján a humán szívizom regionális elektromos inhomogenitásának vizsgálatára a kutyaszívből izolált szívizomsejtek a legmegfelelőbbek.

The aim of our study was to examine the distribution of ionic currents and the concordant channel proteins in the apical (APEX) and basal (BASE) region and the epicardial (EPI) and midmiocardial (MID) layers of the human and canine ventricular myocardium. We compared the data obtained using human and canine cardiac tissue and opposed the electrophysiological and the immunhistological measurements as well. We also examined the effects of testosterone and estrogen on the ECG parameters and on the expression of cardiac ion channels in male and female dogs. Conventional microelectrode whole-cell voltage clamp and Western blot technique were used on isolated cardiomyocytes. ECG records were taken from male and female anaesthetized dogs before and after castration, and following inverted hormone substitution. We found that the duration of the action potential was shorter, the amplitude of the early repolarisation was larger on cells isolated from the APEX and EPI region of the ventricle compared to those of the BASE and MID region. There was no difference on the other parameters of the action potential or on the capacity of the cells. During voltage-clamp measurements the amplitude of the transient outward potassium current (Ito) and the slow component of the delayed rectifier potassium current (IKs) were approximately twice as large on APEX and EPI cells than on BASE and MID ones. The amplitude and kinetic properties of the other examined ionic currents (IKr, IK1, ICa) were similar in cells from all examined origins. The results of the immunhistochemical experiments - consistent with the data of the current measurements - showed more intense apical expression only in the channel proteins of the Ito and IKs: the density of the Kv1.4, KChIP2, KvLQT1 and MinK were higher in apical than in basal samples of canine hearts. During the transmural comparison, only the Kv4.3, Kv1.4, KChIP2 and KvLQT1 proteins had higher expression level in the EPI region than in the MID one. We found similar ion channel protein expression pattern in the human samples than in the canine ones. During the sexual difference investigation we found that heart rate was decreased and PQ interval increased by castration in both genders, while inverted hormonal substitution restored control values. Orchiectomy significantly increased the 55 duration of QT and QTc intervals, QTc-dispersion, while testosterone treatment of castrated females had opposite effects. Expression of ion channel proteins responsible for mediation of IK1 and Ito currents (Kir2.1 and Kv4.3, respectively), was higher in normal males and in the testosterone-treated castrated females. Repolarization of canine ventricular myocardium is significantly modified by testosterone. This effect is likely due to augmentation of expression of K+-channel proteins. The regional and sexual differences found in the canine and human ventricular myocardium are remarkable regarding both dispersion and drug effects. Isolated canine vetricular myocytes are appropriate for the study of the electrical inhomogenity of the human myocardium.
