This thesis presents the design and implementation of a user-friendly and secure Patient Health Record System that is built with React/Convex. The system allows individuals to store, review, manage, and have full control over access to their records. A key feature of the platform is that users have the ability to share their records with healthcare providers by creating organizations and assigning roles to them with different levels of authority. The system bridges the gap between Personal Health Records systems(PHR) and Electronic Medical Records systems (EMR), allowing collaboration while maintaining patient ownership of their data. The results highlight how thoughtful system design can empower patients and simplify healthcare data exchange in a secure and transparent way.
