They work by inhibiting the enzyme neprilysin, which increases the levels of beneficial peptides like natriuretic peptides. Neprilysin inhibitors have been shown to improve symptoms, reduce hospitalizations, and increase survival rates in patients with HFrEF. By reducing fluid retention and promoting vasodilation, these inhibitors help to alleviate the workload on the heart and improve cardiac function. Neprilysin inhibitors are often used in combination with an angiotensin receptor blocker (ARB) to maximize their therapeutic effects. Close monitoring of blood pressure and kidney function is recommended during neprilysin inhibitor therapy, as side effects such as low blood pressure and elevated potassium levels can occur.
