Cardiovascular manifestations can involve vessels leading to coronary artery disease or vascular thromboembolism. It can also cause conduction abnormalities and impaired heart function. Echocardiographic changes associated with impaired heart function include systolic and diastolic dysfunction, left ventricular hypertrophy, pulmonary artery hypertension, and wall motion abnormalities. Separation of myositis cardiac involvement and cardiovascular comorbidities is a challenge for clinicians. In this cross-sectional study, forty-five patients with IIM were enrolled. Conventional echocardiographic and tissue Doppler imaging parameters of systolic and diastolic left and right ventricular function were measured. Correlations between echo findings and disease characteristics have been calculated using SPSS 29 software. Mean age 59.5±12 years, disease duration 13.6 (1-36) years; 67% (n=30) polymyositis, and 33% (n=15) dermatomyositis. There were no differences in comorbidities (hypertension, diabetes, etc.) The distribution of EF was the same across subsets of myositis (PM; DM). Based on logistic regression analysis independent risk factors of aortic insufficiency were hypertension (P<0.001) and joint involvement (p:0.049). ILD (p<0.001), arthralgia (p<0.001), corticosteroid (p:0.046), and IVIg (p<0.001) treatment were associated with mitral insufficiency. Age (p:0.034), cyclosporin (P:0.048), and IVIg (p:0.040) treatment have been associated with diastolic dysfunction. Myocarditis (p:0.030), anti-SSA antibody positivity (p:0.021), and necrotizing autoimmune myopathy (p:0.028) are risk factors of hypokinesis. Finally, cancer-associated myositis (p:0.044) and anti-PL7 antibody positivity (P:0.025) have been associated with elevated right ventricular pressure. Our results demonstrate how different echocardiographic changes can be observed in idiopathic inflammatory myopathies. Our findings referring to clinical subsets can have an influence on cardiac alterations. More studies are needed with higher patient populations to prove these observations.
