Endometriosis represents a major challenge in reproductive medicine, affecting approximately 10% of women of reproductive age, and is one of the leading causes of female infertility. Approximately 30–50% of women with endometriosis experience infertility, while 25–50% of infertile women are affected by the disease. This thesis examines the complex relationship between endometriosis and reproductive outcomes by exploring the mechanisms underlying endometriosis-associated infertility, contemporary management strategies, and pregnancy outcomes. It critically evaluates current evidence on surgical treatment, assisted reproductive technologies (ART), and fertility preservation, highlighting their respective benefits, limitations, and implications for ovarian reserve and reproductive outcomes. The findings emphasize the importance of individualized management based on disease stage, ovarian reserve, patient age, and reproductive goals. By synthesizing current evidence, this thesis provides a comprehensive and clinically relevant overview to support evidence-based clinical decision-making and optimize reproductive outcomes in women with endometriosis.
