The rate of caesarean delivery has increased in the last decades and has been a subject of debate for years. Multiple factors have been identified as the cause in numerous researches all around the world and measures to decrease it been suggested or tried. Despite evidence of substantial benefits with CD-rate higher than a certain threshold, the question could be asked, why are we still facing this problem in 2018? The aim of this thesis was to look at trends in Caesarean delivery, reasons for its worldwide increase and possible methods in order to manage its high rate. The findings were surprisingly different trends between countries and even facilities within the same country and alarming evidence for lack of the procedure in low income countries and overuse in countries with higher income. The conclusion is that the complexity of the contributing factors to increased global CD-rates and variation causes it to be a very sensitive and difficult matter. It will take time to see decrease in the rates and it might even only be moderate.
