Restoring blood flow is crucial for tissue survival, yet it can paradoxically exacerbate tissue damage in ischemia-reperfusion injury, a clinically relevant process. Myocardial infarction, stroke, organ transplantation, acute renal injury, hepatic surgery, intestinal ischemia, and limb revascularization are only a few of the acute and surgical disorders that cause it. Reperfusion exacerbates injury through oxidative and nitrosative stress, inflammation, endothelial dysfunction, leukocyte adhesion, vascular leakage, and microvascular no-reflow. During ischemia, oxygen deprivation causes ATP depletion, acidosis, ionic imbalance, mitochondrial dysfunction, and activation of cell death pathways. The primary focus of this thesis is on pharmaceutical approaches that target oxidative stress, reactive nitrogen species, inflammation, mitochondrial dysfunction, endothelial damage, and cell survival pathways in order to lessen ischemia-reperfusion injury. Antioxidant therapy, nitric oxide and reactive nitrogen species modulation, PARP inhibition, carbon monoxide-based cytoprotection, erythropoietin, adenosine receptor modulation, and other cytoprotective medicines are some of these strategies. However, due to the multifactorial and organ-specific nature of ischemia-reperfusion injury, future treatment will probably need mechanism-based, organ-targeted, and customized treatments backed by enhanced drug delivery, biomarkers, artificial intelligence, and omics-based techniques.
