In this study, we evaluated the role of circulating myeloblasts as potential biomarkers in patients with primary myelofibrosis (PMF). We identified and characterized circulating CD117⁺/CD34⁺/CD45dim myeloid precursor cells and examined their association with disease features and outcomes. Higher levels of these circulating cells were associated with more advanced disease characteristics and an increased risk of leukemic transformation. Our findings suggest that circulating myeloblasts may reflect underlying disease burden and progression in PMF. These results support the potential use of peripheral blood myeloblast quantification as a noninvasive prognostic tool. Further studies are needed to validate these findings and determine their role in routine clinical practice.
