A CF a leggyakoribb autoszomális recesszív módon öröklődő genetikai anyagcserebetegség a kaukázusi populációban, amelyben a CFTR ioncsatorna nem működik, vagy nem expresszálódik megfelelően. Mindez a külső elválasztású mirigyek megváltozott, besűrűsödött váladéktermelését eredményezi, amely az érintett szervek progresszív károsodását okozza. A CF-ben alkalmazott új CFTR modulátor terápiák hatékonyságának monitorozására jelenleg elérhető klinikai paraméterek korlátozottak.
Cystic fibrosis (CF) is the most common autosomal recessive inherited genetic metabolic disorder in the Caucasian population, characterized by the absence or dysfunction of the CFTR ion channel. This results in the production of altered, viscous secretions by exocrine glands, leading to progressive damage of the affected organs. Currently available clinical parameters for monitoring the efficacy of novel CFTR modulator therapies in CF are limited. Airway inflammation characteristic of the disease can be detected even at early stages; therefore, measuring the concentrations of mediators involved in these processes in blood or airway samples represents a feasible approach for assessing therapeutic effectiveness. These therapies improve lung function, reduce inflammation, and decrease the frequency of infections. However, there remains a need to identify reliable biomarkers that enable the monitoring of disease progression and treatment efficacy. In summary, plasma and serum HE4 concentration correlates with improvements in pulmonary function in CF and may serve as novel, reliable, and easily measurable biomarker for monitoring the efficacy of both Orkambi® and Kaftrio® CFTR modulator therapies. Moreover, our findings indicate that EMT-induced downregulation of HE4 expression is accompanied by an increase in MMP9 levels in CF airway epithelial cells, and decreased HE4 expression contributes to the development of an EMT-associated phenotype and reduced cell proliferation.
