dc.description.abstractA COVID-19 világjárvány során az egészségügyi erőforrások végessége felhívta a figyelmet a rizikóbecslés fontosságára. Tudományos munkám során COVID-19 tüdőgyulladás miatt hospitalizált betegeken vizsgáltam a prognosztikai faktorok értékét és a tocilizumab (TCZ) terápia hatását a klinikai kimenetelre. A SARS-CoV-2 okozta tüdőgyulladás súlyosságának és prognózisának meghatározására több vizsgált demográfiai, klinikai és laboratóriumi paraméter is alkalmasnak bizonyult. A közösségben szerzett tüdőgyulladás rizikóbecslésére kifejlesztett A-DROP pontrendszer jó prognosztikai értéket mutatott a COVID-19 pneumóniás betegek súlyosságának és kimenetelének előrejelzésében. A mellkas CT mintázatok, valamint a Pan és munkatársai által javasolt mellkas CT súlyossági pontrendszer szintén hasznosnak bizonyultak a betegség kimenetelének becslésében és korrelációt mutattak több klinikai és laboratóriumi biomarkerrel is. A kortikoszteroid kezeléshez adott TCZ terápia biztonságosnak bizonyult és javította a súlyos, multiszisztémás gyulladásos szindrómával társuló COVID-19 pneumóniás betegek túlélését és csökkentette a gyulladásos markerek szintjét. A kezelés eredményességét javította, ha még a preintenzív fázisban indították, illetve történt konzultáció a kezelésben jártas szakemberrel.
dc.description.abstractDuring the COVID-19 pandemic, the limitations of healthcare resources highlighted the importance of risk assessment. In my scientific work, I examined the value of prognostic factors and the effect of tocilizumab (TCZ) therapy on clinical outcomes in patients hospitalized with COVID-19 pneumonia. Several demographic, clinical, and laboratory parameters proved suitable for determining the severity and prognosis of pneumonia caused by SARS-CoV-2. The A-DROP scoring system, developed for risk assessment of community-acquired pneumonia, showed good prognostic value in predicting the severity and outcomes of COVID-19 pneumonia patients. Chest CT patterns, as well as the chest CT severity scoring system proposed by Pan and colleagues, also proved useful in estimating disease outcomes and correlated with several clinical and laboratory biomarkers. TCZ therapy added to corticosteroid treatment was found to be safe and improved survival in severe COVID-19 pneumonia patients with multisystem inflammatory syndrome, and it reduced levels of inflammatory markers. The effectiveness of the treatment was enhanced when initiated during the pre-intensive phase and when consultation with a specialist experienced in the treatment was conducted.
