As stated by the World health organization (WHO), the disease has claimed over 39 million lives worldwide [1]. Acquired Immunodeficiency syndrome (AIDS) was first recognized in 1981 among homosexual men that presented with rare opportunistic infections and malignancies. The causative agent HIV was later discovered in 1983 [2]. HIV belongs to the genus Lentivirus of the Retroviridea family. The virus is mainly transmitted via unprotected heterosexual or homosexual contact, vertical transmission from mother to fetus or newborn, shared intravenous drug apparatus, and through contaminated blood and blood products. HIV has two main subtypes; HIV-1 and HIV-2, of which HIV-1 is more virulent. HIV-1 is believed to have originated from cross species transmission from chimpanzees in Central Africa while HIV-2 has been linked to simian viruses that infect sooty mangabeys in Western Africa. Both subtypes have a similarity in their structural gene organization, and replication cycle. However, HIV-2 is less pathogenic and tends to cause a much longer latency period [3]. HIV-1 has a higher risk of transmission, is more aggressive, and has a faster rate of progression to AIDS, while HIV-2 has a lower risk of transmission, is less aggressive, and is associated with a longer latency period and slower progression to AIDS [3]. It is believed that HIV evolved from Simian Immunodeficiency Virus (SIV) as a result of in-host viral recombination. HIV-1 descended from SIV chimpanzee; a strain of SIV while HIV-2 has diverged from SIV sooty mangabey variant [4]. Theories have been proposed explaining how HIVs were transferred from animals to humans. The hunter theory is the most accepted theory; in which it is implied that the virus was transmitted during hunting activities, through cutaneous exposure to infected primate blood. Another rather controversial theory is the oral polio vaccine theory that suggests that the virus was transferred iatrogenically via polio vaccines developed in infected cells. The colonization theory suggests that during the colonial era the colonizers may have been infected by the virus and helped spread the virus worldwide by bringing souvenirs and slaves at the end of the colonial era [4]. Over 95% of pediatric HIV infections are acquired via mother-to-child transmission (MTCT) especially during labor and delivery [5]. The introduction of antiretroviral therapy (ART) and alternative mode of delivery via caesarean section has led to successful reduction in perinatal transmission of HIV. Postnatal transmission via breastfeeding is also implicated; hence the recommendation that breastfeeding should be avoided. Certain maternal and obstetric factors increase the rate of HIV transmission such as: prematurity, advanced maternal disease, and high maternal viral load [5]. Thanks to easy access to anti-retroviral drugs in developed countries, there has been a remarkable reduction in MTCT; new pediatric infections are more common in developing countries where antenatal screening is not comprehensive and interventions to reduce MTCT are not widely offered. [6] The aim of my thesis is to discuss AIDS in the pediatric population and also to briefly brush on the role of sexual abuse in the transmission of the infection. The thesis discusses and proposes some of the proteins that interact with vpX to further understand the its role in the prolonged latency period of HIV-2. The methods used to examine this were Proximity Ligation Assay, co-Immunoprecipitation, and Western Blot. Through the PLA, it was seen than CTBP-2, a known co-repressor, interacts with the vpX. For this, bone marrow and peripheral blood samples were collected from eight patients. Mononuclear cells were isolated and counted, then, to analyze cells, colony Forming Cell (CFC) assay was performed and colonies were counted and identified. We performed 14 CFC assays for eight cases. Colonies grew successfully for three patients and did not grow from the remaining five patients. A klinikai adatok alacsonyabb fertőzőképességet és sokkal lassabb progressziót mutattak a betegség kialakulásában, azokban a betegekben, akik mind a két vírussal fertőzöttek. Előző kutatásaink azt mutatják, hogy a HIV-2 gátolja a HIV-1 replikációját a 293T HEK sejtekben. Ez a hatás valószínűleg a HIV-2 egyik tartozék fehérjéjének a virális protein X-nek köszönhető (Vpx). Célunk az volt, hogy tanulmányozzuk a kettősfertőzés hatásait az immunsejtekben, mint például THP-1 monocita, és Jurkat T-limfocita. Második generációs lentivirális vektorokat használtunk, HIV-1 és HIV-2 pszeudovirionok létrehozásához, valamint egy vektort amely HIV-2 Vpx-et kódol. A THP-1 sejteket aktiváltuk forbol 12-mirisztát 13-acetáttal (PMA), majd elvégeztük a kettősfertőzésre alkalmas kísérleteket. Előző eredményeink azt mutatták, hogy amikor a kettősfertőzés modellt felállítottuk a THP-1 sejtek esetében, nem tapasztaltunk szignifikáns változást a HIV-1 fertőzőképesség csökkenésében. Hipotézisünk az volt, hogy azért nem történt HIV-1 gátlás, mert a THP-1 sejtek felszínén található SAMHD-1 protein csökkentette a Vpx mennyiségét. Ennek következtében a sejteket aktiváltuk PMA-val, és transzfektáltuk GFP-Vpx kódoló konstrukttal, annak érdekében, hogy minimalizáljuk a SAMHD-1 szintet a THP-1 sejtek felszínén. Ezt követően azt tapasztaltuk, hogy a Vpx transzfekciója csökkentette a HIV-1 transzdukciós képességét. A HIV-1 pandémiás, míg HIV-2 főként Nyugat-Afrikában van jelen. 1988 óta ismert a HIV-D, azaz a HIV1/2 kettősfertőzés is, amely azt jelenti, hogy az adott beteg mind HIV-1 mind pedig HIV- 2 vírussal fertőzött. Ezen típusú fertőzés jellegzetesen olyan területeken fordul elő, ahol egyébként is jellemző mindkét vírus jelenléte. Travers és munkatársai voltak azok, akik először felismerték, hogy HIV-D fertőzés esetén a HIV-2 képes valamilyen módon természetes védelmet nyújtani a HIV-1 vírussal szemben, amelynek eredményeként a HIV-D fertőzött egyedeknek a túlélési ideje hosszabb, és jobb a prognózisuk, ezáltal sokkal lassabban alakul a fertőzés AIDS betegséggé. A jelenleg ismert gyógyszercsoportok: nukleozid reverz transzkriptáz inhibitorok (NRTI), proteáz inhibitorok (PI), nem nukleozid reverz transzkriptáz inhibitorok (NNRTI) fúzió inhibitorok (FI), CCR5-inhibitorok, integráz inhibitorok(II). Such data are really scarce, and given the ever increasing incidence of HIV-2 infection, it is wise to characterize the viral responsiveness to combination therapies containing PIs. Having optimized the expression and stability of the viral protease, we can conclude from our comparative enzymatic and cell culture inhibition profiling that lopinavir, darunavir, indinavir sulfate and saquinavir are indeed highly potent HIV-2 protease inhibitors. Tipranavir and nelfinavir on the other hand, showed a significantly lower efficacy when compared to the others. While amprenavir consistently was proven to be a weak inhibitor, the variable results obtained for atazanavir sulfate; enzymatically and in cell culture, incline us to doubt its reliability when used in HIV-2 infected patients. I54M-L90M mutations reduced the susceptibility of the protease to most of the inhibitors to variable degrees, with the exception of tipranavir. Perhaps the use of tipranavir in patients with such identified mutations is advantageous after all.
